Personal data the business holds (as defined in the Act on the Protection of Personal Information) retained by Idemitsu The same applies hereinafter.) We will respond to any of the following requests made regarding personal information or requests for disclosure of records provided to third parties (“Requests“) in accordance with the paragraph 5-2, 5-3, and 5-4.

(ii) Correction, addition to, or deletion of content (the collectively  “Correction, etc.“)
(iii) Suspension of use or deletion (collectively the “Suspension of Use, etc.“)
(iv) Disclosure of personal data the business holds and records of provision to a third party

（1）ご請求のお申し出先
お客様が個人情報を提供された当社の各サービス、サイト等が個別に設置するお問い合わせ窓口まで、お申し出ください。お問い合わせ窓口が不明な場合、 こちら をご確認ください。

なお、当社の会員制ウェブサービス、通信販売サイト等でご登録された会員情報、お客様情報等は、お客様ご自身で確認、変更、消去等が可能な場合があります。また、個人情報の利用目的は、本プライバシーポリシー等に記載されている場合があります。これらの場合、お客様ご自身でご確認・ご対応いただくようお願いいたします。

（2）ご請求の方法
①保有個人データの開示請求または第三者提供記録の開示請求の場合

上記（1）のお問い合わせ窓口にてご案内する住所宛てに、次の必要書類を郵送することにより、お申し出ください。なお、封筒に朱書きで「個人情報に関する請求申込書類在中」とお書き添えください。ご請求１件につき、1,000円分の手数料（回答送付にかかる実費相当分の郵便切手。ただし、回答方法または回答内容の分量に応じ、別途手数料の額および支払方法を定めることがあります。）をお支払いいただきます。

＜必要書類等＞
請求申込書を開く

・運転免許証、健康保険の被保険者証、パスポート、マイナンバーカード、年金手帳、特別永住者証明書のうち、いずれか１点の写し（代理人からのご請求の場合、ご本人および代理人の両名分）
・（上記の本人確認書類に現住所の記載がない場合）上記の本人確認書類に加え、住民票または公共料金の請求書であって、ご本人（代理人からのご請求の場合、ご本人および代理人の両名分）の氏名および現住所が確認できるもの１点の写し
・（代理人からのご請求の場合のみ）委任状等、ご本人との代理関係を確認できる書類
・1,000円分の郵便切手（ただし、別途手数料の額および支払方法を定めることがあります。）

*We will notify you if any of the above required documents are incomplete or missing. However, if the required documents are not completed within two weeks after notification, we will deem that you have not made the request. Please be aware that we may contact you by telephone or other means to confirm the details of your request.
*If the prescribed fee is not paid after notification, we will treat the request as if it had never been made. Please note that the fee will not be refunded regardless of the reason.

(ii) For requests for the Correction, etc., the Suspension of use, etc., suspension of provision to a third party, or notification of the purposes of use of personal data the business holds

Please inform us of the details of your request at the inquiry desk in (1) above and provide us with the information necessary to verify your identity, such as name, address, date of birth, and telephone number of the requester, in accordance with the guidance. As in the case of a request for disclosure of personal data the business holds or a request for disclosure of records of provision to a third party, we may ask you to submit documents verifying your identity. In the case of a request from a representative, please provide us with documents that can verify the representative’s relationship with the requester (or other equivalent method if it is difficult to provide such documents).

(3) Method of verifying the identity of the person concerned and his/her representative
We will confirm your identity and the identity of your agent by checking the information provided necessary to confirm your identity and the identity of your representative and the contents of the documents confirming your identity and your relationship with your representative.

(4) Method of response to requests
We will respond to requests for disclosure of personal data the business holds or requests for disclosure of records of provision to third parties by sending a written document to your current address indicated in the required documents in (2) above or by sending an electromagnetic record as an e-mail attachment to the e-mail address indicated in the request application form in (2) above, in accordance with the request of the requester. However, if disclosure by the latter method requires a considerable sum of money or is otherwise difficult, we will respond by the former method after the notification to that effect.

Requests for the Correction, etc., the Suspension of use, etc., suspension of provision to a third party, or notification of the purposes of use of personal data the business hold will be responded to in the manner prescribed by Idemitsu.

Personal information acquired in connection with Requests will be used only for the purpose of investigating the request, confirming the identity of the person concerned or his/her representative, sending a response, and making necessary contact. Documents sent to us will be destroyed within 6 months after the completion of the response.

If any of the following items applies, we will not respond to the request and will notify the applicant to that effect.

(1) If the identity of the applicant cannot be verified
(2) If the right of representation cannot be verified where a request is made by a representative
(3) If the subject of Requests does not fall into the category of personal data the business holds
(4) If the Correction, etc. is not necessary in view of the purpose of use, or when it is incorrectly claimed that the information is erroneous.
(5) If there is no reason for the request for the Suspension of use, etc.
(6) If provision to a third party is made without violating the Act on the Protection of Personal Information.
